LJM Memorial Hospice

LJM Memorial Hospice provide community engagement and support services to improve access to care and ensure that people are not alone during the life’s most difficult moments. Their main mission is ” A community for life’s final chapter”. Their main focus is to improve end-of-life care support. They work to support individuals and families who face illness and grief by providing community connection and emotional support. They run community programs, and activities for people to talk, plan and feel supported through end-of-life and palliative care. They host events designed to open hearts, build understanding, and create connection. All LJM Memorial Hospice’s events and workshops are designed to help people feel less alone and more supported. Their long term goal is to establish a local hospice facility so that people can access high quality end-to-life care closer to home.

LJM Memorial Hospice will use funding to improve their community activities and expand the ability to provide better connection at the end-of-life care. This includes conducting more community events, and support will help them to reach more people who are in need of emotional support.
